Urology Flashcards
urethral injury
Hx of pelvic fracture
Urinary retention, blood at the urethral meatus and a high riding prostate on digital rectal examination
Tx= suprapubic catheter
complications of radical prostatectomy
incontinence, erectile dysfunction and urethral stenosis
muddy brown casts
ATN
non-seminoma germ cell testicular cancer tumour markers
AFP, HCG
seminoma germ cell testicular cancer tumour markers
HCG
Hydrocele investigation
Testicular USS to rule out testicular cancer
symptomatic renal cell carcinoma stage? and Tx
stage 4
pembrolizumab and axitinib
complications of TURP
T urp syndrome
U rethral stricture/UTI
R etrograde ejaculation
P erforation of the prostate
TURP syndrome
- Hyponatraemia: dilutional
- Fluid overload
- Glycine toxicity
‘small dribbles of urine’ passing when he does not want them to + Hx of gonorrohoea
urinary stricture
Low pressure chronic urinary retention
painless distended bladder, but no associated hydronephrosis or renal impairment.
High pressure chronic urinary retention
renal function is impaired or if there is hydronephrosis
most common organic cause of ED
vascular:
cardiovascular disease (CVD), hypertension, hyperlipidaemia, diabetes mellitus, smoking, and major pelvic surgery.
BPH managed w tamsulosin and finasteride, ongoing urinating issues
add anti-muscarinic for overactive bladder e.g. oxybutynin, tolterodine
decompression haematuria
following catheterisation for chronic urinary retention
does not require further treatment if haemodynamically stable
hydronephrosis investigation
USS of renal tract
prostatitis Tx
quinolone for 14 days
e.g. ciprofloxacin or ofloxacin
partial vs radical nephrectomy
T1–> partial
T2 (>7cm)–> radical
Periureteric fat stranding may indicate
recent stone passage
first management in hydronephrosis
remove obstruction and drainage of urine (catheter)
renal colic- analgesia if NSAIDs contraindicated
IV paracetamol
A pelvic fracture and highly displaced prostate
membranous urethral rupture
A pelvic fracture and lower abdominal peritonism
bladder rupture
radiolucent stones
uric acid
