Ureter, Prostate and Bladder Flashcards

1
Q

What are the different parts of the male urethra?

A

Preprostatic urethra - between bladder and prostate.
Membranous urethra - Passes through the perineal membrane.
Spongy urethra - from below urogenital diaphragm to the tip of the penis. (through spongey tissue of the penis)

Q

Compare the urethral sphincters in males and females.

A

Females: only one external sphincter.
Males: 2 urethral sphincters.
- one in deep perineal pouch.
- one at neck of the bladder (internal sphincter) to prevent back flow of semen into the bladder during ejaculation.

Q

Outline the pelvic course of the ureters in males and females.

A

Males: Over the external iliac artery, Over the umbilical artery, Under the seminal vesicle and Vas Deferens.
Females: Over the external iliac artery, Over the obliterated uterine artery, Under the Uterine artery

Q

Where do the ureters connect to the bladder?

A

Vesico-uteral junction - on the posterior wall of the bladder.
Forms the trigone base of the bladder with the urethra.

Q

What is the name of the muscle lining the bladder? What is its function?

A

Detrusor muscle - lines bladder but not over base.

  • Folded wall to enable expansion of the bladder.
  • Contracts during micturition

Q

What is hydronephrosis?

A

When fluid backtracks up the ureters to the kidney.
Causes swelling of the kidney.
Occurs due to a defective valvular system.

Q

What are the relations of the male bladder?

A

Q

What are the relations of the female bladder?

A

Q

What is the arterial supply of the bladder?

A

Two arteries:

  • Superior vesical artery - branches off the internal iliac.
  • Inferior vesicle artery (vaginal artery in females) - branches off the anterior division of the internal iliac.

Q

What is the lymphatic drainage of the ureter (upper, middle, lower)?

16
Q

What is the lymphatic drainage of the bladder?

A

To the internal iliac lymph nodes.

17
Q

Outline the parasympathetic innervation of the bladder.

A

Parasympathetic pelvic nerves arise from S2,3,4.
Excitatory to the bladder (contracts detrusor muscle)
Relaxes the sphincter

18
Q

Outline the sympathetic innervation of the bladder.

A

Via the hypogastric nerves to pelvic ganglia. Arise from T10-L2.
Tonic contraction of the spincter and relaxation of detrusor muscle.

19
Q

Outline the somatic innervation of the bladder.

A

Somatic nerves S2-4 - pudendal nerve.
Excitatory to the external sphincter.
Voluntary contraction of external sphincter to control micturition.

20
Q

What innervation occurs during micturition?

A

Switch off sympathetics and somatics.

Switch on parasympathetics via pelvic nerves.
