Upper Limb Bracing Flashcards
Shoulder elbow wrist orthosis (airplane SEWO)
fx: transmits weight of upper limb to pelvis
idc: rotator cuff repairs, AP cap repairs, axially burns,
Shoulder elbow orthosis (gunslinger orthosis)
fx: supports painful shoulder or brachial plexus injury. . Places hand in position for functional activities.
Inc: brachial plexus injury, subluxing glenohumeral joint.
Antideformity position of the hand
slightly more wrist extension, MCP flexion and palmar abduction, but fingers straight.
Short Opponens
fx: maintain hand in functional position. Thumb in opposition and abduction.
No orthotic wrist control.
Indications:
Inflammation of thumb
median nerve lesion
C6-C7 SCI
Ratchet wrist hand orthosis
fx: Wrist position can be changed for different activities, . Achieve pinch by pressing nob attached to device with chin or other hand.
idc: total paralysis or severe weakness of hand and wrist. Must have at least poor shoulder and elbow control. C5 quad.
Elbow orthosis
Fx: reduce soft tissue contractors. Incorporates at least one mechanism of increasing range of motion.
Ind: SCI
When to apply an Outrigger bar to a WHO
Combination of MCP extension stop with PIP extension assist to prevent claw hand.
Wrist Driven wrist hand orthosis (WDWHO)
Uses functional tenodesis to allow for grasp.
Indications: C6-C7 SCI with 3- to 3+ wrist extensor strength.
Resting hand splint
fx: keeps hand fixed in a C shaped pattern to prevent contractors and maintain webspace. It can also reduce pain and inflammation.
Idc: Flaccid hand, burns, trauma to hand or wrist.
3 different types of orthoses
Immobilization
Mobilization
Restriction
Functional position of the hand
20-30 degrees of wrist extension
40 degrees of MCP flexion
45 PIP
Thumb in palmar abduction
Wrist Hand Orthosis
fx: same as short opponens with additional wrist stabilization.
Indications: Dequervain's tenosynovitis median and ulnar nerver lesions C5-C6 lesions Pathology of wrist thumb joint. Scaphoid or bennett's fx.
