Purpose of bracing
Substitute for muscle weakness
Control spasticity
Maintain skeletal alignment and prevent deformities
Sagittal and coronal plane control during gait
When to use PF/DF stops
DF stop- provides control of dorsiflexion in loading response to substitute for weak plantar flexors and controls knee flexion control.
PF stop- Keeps foot from dropping due to weak DF. Limits knee hyperextension in stance.
Be careful of knee instability during loading response
SAFO indications
fair quad strength
combined PF and DF weakness
Best ML control
PLS AFO
Controlled PF at loading response
Allows for DF
For individuals with weak DF.
Provides minimal ML support.
GRAFO
Indicated with quad strength 3-/5
Provides knee extension moment at terminal stance.
Align at tibial angle of 90 or slightly plantar flexed
Longer foot plate will increase moment.
Cavus Foot
Loss of intrinsic foot muscles.
Toe flexors intact.
Pes planus
Tight achilles
Post tib dysfunction
Unopposed strength of peroneals
Plantarflexion contracture
Unopposed PF.
Spasticity
PF 1st ray
Peroneus longus over contracting
