What is Charcot Foot?
Impaired pain and proprioception in foot
Loss of motor and sensory function allows micro traumas that turn into deformity after going undetected.
Early Stages of Charcot Joint
Swelling, dislocation and instability of torso-metatarsal joint, subluxation. Crepitus from maligned bones grinding on each other.
Charcot Complications
Calluses and ulcers
Infection may develop
Goal of treatment
Distribute plantar pressures
Stabilize Joint
Treatment
Surgical- osteotomy or realignment
Nonsurgical- Total contact cast or CROW boot and crutches. Stages 1 and 2. Maybe stage 0
After resolution of process- custom shoes and FOs. Usually in stage 3
Total Contact Cast
Cast change every 5-7 days
Contraindications- Severe edema, thin skin, active infection.
CROW boot
- ) cast in neutral position.
- ) correct varus or valgus deformity
- ) rocker bottom sole and custom arch support.
