Upper airway disease Flashcards
UAD
Resp distress. Epiglotitis (H influ), Pertussis (bronchioles and trachea), Croup (obs larynx - parainfluenza), RSV (Bronchiolitis)
H Influenza Clin
epiglotitis with thumb sign. Fever and pharyngitis. Stridor
H Influenza Char
Gram neg rod, PRP capsule, requires X and V factors
H Influenza Diag
Lateral X-ray of neck for thumb sign
H Influenza Tx
Hib vccn, rifampin to eliminate carriage
Bordetella Pertussis Clin
Whooping cough
Bordetella Pertussis Char
gram neg rod
Bordetella Pertussis Diag
Bordet-Gengoi or Regan lowe agar, PCR
Bordetella Pertussis Vccn
DaPT
Bordetella Pertussis Stages
- Catarrhal (1-2 wks) = cold with fever
- Paroxysomal (2-4 wks) - destruction of ciliated epi
- Convalescent - secondary complications = Pneu, seizure, encephalitis
Bordetella Pertussis VF
Filamentous Hemaglutinin, Pertactin, Pertussis toxin (PTx/ACT increases cAMP leading to edema, inhibs chemokine receptors, activates islet cells leading to hypoglycemia and increased histamine sensitivity)
Parainfluenza Clin
Croup = barking cough
Parainfluenza Char
ss neg sense RNA with envelope, H and I peplomers
Parainfluenza Diag
Frontal X-ray with steeple sign
Parainfluenza Tx
Dexamethasone/ epi to aid resp
