Orthomyxoviridae Pathogenesis
Local URT infections kills mucous secreting cells and cilia. NA cleaves mucous allowing more cell infections. Viremia and subsequent cytokine release leads to myalgia. If to lungs, causes desquamination of epithelium in bronchi and alveoli leading to susceptibility to bact pneumonia.

Enveloped segmental neg sense RNA (8 segments). A = epidemic/ pandemic; B = major outbreaks; C=mild

HA (16) binds to cell surface receptor to initiate infiltration
NA (9) cleaves N-acetyl neuraminic acid to release virus and nucleus
Orthomyxoviridae Diag
Cell culture, nasal swab for rapid Ag detect (EIA and DFA), serum abs by HAI and EIA for typing. CXR for interstitial pneumonia
Orthomyxoviridae Tx
A = Admantin antivirals = Amantidine and Rimantidin: viral M2 ion channel inhib leading to loss of coat
A/B = Tamiflu (oseltamivir) = NA inhib

Inact vccn - intra musc, low IgG titers induced - short
Live attenuated = Flumist for non pregnant. Induce IgA via temp sensitive A and B

Reassortment of segmented genome ( HA and NA change). M1 and M2 conserved
