Fungal infection most common in immunocomp. Dimorphic with hypha at 25C and yeast at 31C
Histoplasma Capsulatum Clin
Histoplasmosis (atyp pneu) with lung cavitations which once heal calcify leading to coin lesions. Can be chronic with underlying disease present, can disseminate to liver, spleen, marrow, lymph.
Histoplasma Capsulatum path
Inhaled as hyphae (bird/bat guano), periph blood shows monocytes with intracell yeast. (lung = alv macrophages)

Culture for dimorphic, serology, HPA, Urine Ag, CXR lesions, BAL biopsy, skin test for epidemiology

Blastomycosis (typ pneumonia) with dissemination. Low lobar consolidation, extra pulmonary lesions (include skin ulcerated granulomas).
