What is most causative agent in UTIs?
E coli 85-90% of the time
What are 3 symptoms of UTI?
urgency, frequency and dysuria
What are “uncomplicated” UTIs?
in premenopausal, sexually active females, not prego and no recent UTI
what are “complicated” UTIs?
post menopausal or prego female w/ h/o recent UTI, catheter, or GU defect. OR male
What is first line therapy for UTI?
3 day course of Bactrim
What is 2nd line for UTI?
flurooquinolones 3 days
How is bacterial vs nonbacterial prostatitis diagnosed?
bacterial w/ positive culture. nonbacterial w/ neg culture and positive WBCs.
What is first and second line for prostatitis?
1st: fluoroquinolones or bactrim. 2nd-doxycycline, azithromycin up to 12 wks
What are 5Alpha reductase inhibitors? What are the side effects?
proscar (finesteride) and Avodart (distasteride). Side effects are decrased libido, impotence
When are 5Alpha reductase inhibitors superior?
in larger prostates
What are alpha blockers? what are side effects?
terazosin, doxazosin, tamsulosin. decreased bp, fluid retention
what is first, second line to treat BPH?
1st: AUA score of 7. Alpha blockers OR 5ARIs
What is 1st and 2nd line tx for ED
1st: lifestyle, modify possible causes and start PDE5. 2nd: referral to urology
What do high fat meals potentially do to PDE5s?
decrease absorption
What are anticholinergics or anti muscarinics in OAB? when are they contraindicated?
oxybutynin etc. Contraindicated in narrow angle glaucoma
