Definition class II incisors?
Lower incisor tip occludes behind cingulum plateau of upper incisor
Div I - upper incisors normal or proclined
Div II- upper incisors retroclined
What do you tend to see with class II div I?
Generally see increased overjet
What is limitation for tx of class II cases?
Skeletal pattern
Tx options for class II div I?
- Accept mild overjet- pt appearance may be acceptable
- Removable appliance - used less
- Fixed appliance - can’t correct skeletal discrepancies
- Functional appliance
- Orthoganathic surgery - only severe class II in non-growing pt
FOR FA
Difference tx in growing vs non-growing pt?
Growing - functional appliance and orthodontic camouflage
Non-growing - orthodontic camouflage/ surgery if severe
Best tx for overjet?
SAW - poor at reducing
Functional appliance - excellent reducing
Tx moderate- severe class II?
- Change skeletal pattern
- Functional appliance - if growing
- Surgery -non-growing
Tx mild class II?
- Accept
- Removable appliance
- Fixed appliance
How can orthodontic camouflage be undertaken?
Use anchorage - control unwanted tooth movement
Need careful planning
Loss anchorage - lack of occlusion in buccal segment/ residual overjet
Tx if no lower arch crowding and class I molars?
Non-extraction
Anchorage reinforcement
Tx if no lower arch crowding but 1/2 class II molars
Distal movement of buccal segments
Consider upper extractions (premolars)
Tx if no lower arch crowding but class II molars
Upper premolar XLA
Tx if lower arch crowding and class I molars?
Upper and lower XLA
Tx if lower arch crowding and 1/2 unit class II molars
Upper and lower XLA - correct molar relationship –> allow lower molars to drift forwards
Tx if lower arch crowding and class II molars
Difficult case - avoid in general practice
