Trauma articles Flashcards
Epidemiology, clinical management, and outcomes of dogs involved in road traffic accidents in the United Kingdom (2009–2014)
Harris et al, jvecc 2018
Prevalence of RTA was lo (0.41%) Younger dogs (<3 years vs 6-9y) and male dogs had incr odds
Performance evaluation and validation of the animal trauma triage score and modified Glasgow Coma Scale with suggested category adjustment in dogs: A VetCOT registry study
Kristian Ash et al, jvecc 2018
Mortality 7.3%
Every 1 point increase in ATT increased mortality odds by 2.07
Eye/muscle/integument categories showed poor discrimination
mGCS only good if head trauma and motor component is most predictive
ATT overall performed better
-Can only use perfusion, resp, and neuro categories
Evaluation of the agreement between focused assessment with sonography for trauma (AFAST/TFAST) and computed tomography in dogs and cats with recent trauma
Walters et al jvecc 2018
Agreement b/w aFAST-CT: Free peritoneal fluid»_space; pleural effusion
POOR agreement for pneumo – tFAST not reliable for this
tFAST better to identify fluid than air
Clinicopathologic abnormalities associated with increased animal triage trauma score in cats with bite wound injuries: 43 cases (1998-2009).
Bridget M Lyons, et al jvecc 2019
Outdoor male cats more common
Lower venous pH, higher plasma lactate, and low iCa were associated w/ higher ATT
Compartment syndrome of the muscles of mastication in a working dog following a traumatic training incident.
Amy L Brida, et al jvecc 2019
Emergent decompressive fasciotomy needed
exertional rhabdomyolysis
The pathogenesis of muscle injury is thought to involve both direct injury to the sarcolemma and an inability of cellular ATP production to keep up with demand. Breakdown of muscle fibers results in leakage of toxic cellular contents including myoglobin, creatinine, potassium, CK, and other muscle enzymes.
Association between computed tomographic thoracic injury scores and blood gas and acid-base balance in dogs with blunt thoracic trauma.
Robert M Kirberger, et al jvecc 2019
PaO2 lower in trauma group
-alveolar ventilation spared but clinically sig. V/Q mismatch is present
blunt thoracic trauma can cause sig. pulmonary and pleural injury and blood O2 economy is affected
Successful management of catastrophic peripheral vascular hemorrhage using massive autotransfusion and damage control surgery in a dog.
Rita D K Ghosal and Alexandra Bos. jvecc 2019
58% blood volume transfused back from clean (but not sterile) suction canisters and clots skimmed off –> given back via blood filter
hemoglobinuria developed but resolved within 18h
Performance evaluation and validation of the Animal Trauma Triage score and modified Glasgow Coma Scale in injured cats: A Veterinary Committee on Trauma registry study.
Janis Lapsley, et al jvecc 2019
Overall mortality 16.5%
1 point ATT score incr associated w/ mortality odd incr by 1.78.
Eye/muscle/integument categories lowest discrimination- no change to ATT if removed
mGCS best restricted to head trauma patient, motor component most predictive, full score the best
no diff b/w mGCS and ATT for head trauma
Retrospective evaluation of serum/plasma iron, red blood cell distribution width, and nucleated red blood cells in dogs with acute trauma (2009-2015): 129 cases.
Eric J Fish, et al jvecc 2019
nRBCs and low serum/plasma iron are associated with nonsurvival. RDW was not associated with survival
ABSENCE of hypoferremia = survival.
Nonsurvival group older, more severely injured (APPLE and ATT score)
Utilization of whole body computed tomography in polytrauma patients
Emily T. Dozeman et al jvecc 2020
Common injuries: head, lungs, pelvis
25% had cavitary effusion not found on FAST scan
Association between cardiac troponin I concentrations and electrocardiographic abnormalities in dogs with blunt trauma
Allison A. Biddick et al jvecc 2020
10% developed arrhythmia
Normal initial ECG + normal CTnI both had 100% NPV for ruling out development of CSCA; ECG itself was 95%
Survival to discharge 89%
Trauma-induced primary hypoparathyroidism following severe bite wound injury to the neck in a dog
Johanna Wolf DVM et al jvecc 2020
Chihuahua got bit in the neck, exposing trach + jugular vein, missing R thyroid gland –> 48h after initial trauma came back laterally recumbent, in shock, severe ionized hypocalcemia –> IV calcium gluconate, calcitriol then long-term calcium carbonate + vitD3 –> 6 months later weaned off
1st report of traumatic hypoparathyroidectomy
Diagnosis of pulmonary contusions with point-of-care lung ultrasonography and thoracic radiography compared to thoracic computed tomography in dogs with motor vehicle trauma: 29 cases (2017-2018)
Samuel A. Dicker et al jvecc 2020
LUS had high Sn for diagnosing contusions compared to CT
TXR failed to identify contusions in 33% of dogs
Clinicopathological abnormalities associated with increased animal triage trauma score in cats presenting for vehicular trauma: 75 cases (1998–2009)
Bridget M. Lyons, et al 2020 jvecc
Outdoor male cats overrepresented
Low TP, low venous pH, high lactate, low HCO3, low BE , high BG, low NIBP were associated w/ higher ATT
Shock index is associated with mortality in canine vehicular trauma patients
Mara N. Kraenzlin, et al jvecc 2020
SI higher in those who died and correlated to ATT (1 vs 0.75)
