ATNR: what is the normal response?
No reaction to head rotation
ATNR: what is the abnormal response?
- Extension on face side or increase in extensor tone
- Flexion on skull side or increase in flexor tone
STNR: What is the normal response?
No reaction to neck ext/flexion
STNR: What is the abnormal response?
- Neck extends: arms extends or increase extensor tone, legs flex or increase flexor tone
- Neck flexes: arms flex or increase in flexor tone, legs extend or increase extensor tone
ATNR: What are the 3 challenged positions?
- QP (turn head 4 times)
- QP RIP
- Schiller’s arm extension
ATNR: How do you test QP (turn head 4 times)
What is the response?
Rotate head
Elbows SLIGHTLY flexed, fingers straight ahead
Up to age 5, max 45 deg
Up to age 7, max 30 deg
(If above these deg, it is abnormal)
ATNR: How do you test QP RIP?
What is the response?
One hand on hip, opposite leg extended
90% of 5 years of children can assume and maintain position
ATNR: How do you test Schiller’s Arm extension?
What is the normal finding?
What is a positive response?
Standing, shoulders flexed to 90 deg
Normal finding: 5 yo, max trunk rotation 45 deg
Positive result: shoulder flexion on occipital side, rarely see elbow flexion
