What are the Sulfonamides?
Sulfamethoxazole (SMX), sulfisoxazole, sulfadiazine
MOA sulfonamides
Inhibit folate synthesis via dihydropteroate synthase
Sulfonamide coverage?
G+, G- Nocardia, Chlamydia
SE of Sulfonamides?
Hypersensitivity (allergy), hemolysis if G6PD
Trimethoprim class?
Trimethoprim
MOA Trimethoprim?
Inhibits bacterial DHF reductase
What is Trimethoprim commonly combined with?
SMX
What is TMP/SMX typically used for?
UTIs, Shigella, Salmonella, Pneumocystis jirovecii prophylaxis in immunocomp, toxo prophylaxis
Toxicity of TMP?
Megaloblastic anemia, leukopenia, granulocytopenia
What are the FQs?
Ciprofloxacin (Cipro), norfloxacin (Noroxin), levofloxacin (Levaquin), ofloxacin (Floxin), sparfloxacin, moxifloxacin (Avelox)
MOA FQs?
Inhibit DNA gyrase (topoisomerase II) and topo IV
FQ coverage?
G- rods of UT and GIT (include Pseudomonas), Neisseria, some G+
FQ SE?
GI upset, skin rashes, HA, dizziness; Tendonitis, tendon rupture, leg cramps, myalgias, may prolong QT interval
FQ contraindications?
Pregnant women, nursing mothers, children under 18 yrs (damage to cartilage), > 60yo (tendon rupture)
FQ MOR?
Chromosome encoded mutation inDNA gyrase, plasmid-mediated resistance, efflux pumps
