Fluency ia about ease of speaking: How easily you can connect your sentences:
* Hesitations
* Repetitions: Listen to yourself & try to reduce Hmms Uhmmm as much as pissible
Vocabulary: Range, mistakes, Idiomatic Expressions, Collocations, Paraphrases
Grammer: Try to use wide range of complex grammer structures
Intonation is also important in conveying Rise & Fall

As far as I can see, in my country being a doctor is a popular job, as most of the people want to be rich and make a lot of money. For example when I was in school majority of my class mates teneded to admit in medical majors.
