Components of Therapeutic Relationship

Trust
builds when the nurse conveys integrity and reliability
congruence: words and actions match
- trust is broken when there is no congruence
Genuine Interest
happens when the nurse is comfortable in themselves, aware of their strengths and limitations, and is focused
client can detect when the nurse asks a question but does not really want to know the answer
select use of self-disclosure
Empathy
the ability to put oneself in the shoes of the client
-this does not mean that the nurse has had the exact experiences
reflection, restatement, clarification
Acceptance
the nurse does not become upset or respond negatively with a client who is angry or acting out
avoiding judements
Types of Relationships
social:
- ex) nurse talks to client about weather or sports
- won’t further therapeutic relationship if you stay in a social relationship, but can hep you ease into further relationship
intimate: does not have place in nurse-client interaction
therapeutic: focuses on the needs, feelings, ideas of the client only
- not inserting own feelings, needs, ideas in the relationship
Positive Regard
respect for the client regardless for their behavior, background, or lifestyle
Phases of the Nurse-Client Relationship
- orientation
- working
- problem identification
- exploitation (exploration) - termination
Orientation Phase
begins when the nurse and client meet
ends when the client begins to identify problems and examine
Working Phase
problem identification:
-nurse identifies issues or concerns causing problems
exploitation (exploration):
- nurse guides the client in examining feelings, developing coping skills, etc.
- encourages behavior change
Termination Phase
begins when problems are resolved and ends when the relationship is ended
person is getting very close to discharge
