The Perineum Flashcards
Perineum
Trunk inferior to the pelvis
Diamond shaped
Can be viewed when the thighs are abducted
Perineum is bounded by
Coccyx
Ischial tuberosity
Pubis
A line through the ischial tuberosities divides perineum into
Two triangles
Anal Triangle
Urogenital Triangle
Anal canal can be divided into
Upper and lower halves
Upper half of anal canal
Is above the pectinate line
Derived from endoderm like the rest of the gut tube
Lined by columnar epitheliam
Anal columns present
Innervation of upper half of anal canal
Autonomic - only stretch and pressure receptors (no pain)
Arteries in upper half of anal canal
Superior rectal from IMA
Middle rectal from internal iliac
Veins in upper half of anal canal
Superior rectal - drains into IMV
Lymphatics in upper half of anal canal
Drains to inferior mesenteric nodes
The lower half is
Below the pectinate line
Derived from ectoderm like the skin
Is lined by stratified squamous epithelium
Anal columns are absent
Innervation of lower half of anal canal
Somatic - you feel pain here - hemorrhoids (ouch)
Inferior rectal branch of pudendal
Arteries of lower half of anal canal
Inferior rectal off int. pudendal
Veins of lower half of anal canal
Inferior rectal drains into internal pudendal
Lymphatics of lower half of anal canal
drains to superficial inguinal nodes
Anal Sphincters
Internal and External
Internal anal sphincter
One part
Involuntary
External Anal Sphincter
Three parts (deep, superficial, subcutaneous) Voluntary - innervated by inferior rectal off the pudendal
Ischioanal Fossa Boundaries
Skin, Levator Ani, Obturator Internus, Ischium, Sacrotuberous and sacrospinous ligaments, Deep transverse perineal muscles
The anterior recess of the ischioanal fossa extends into the
UG triangle
Contents of the Ischioanal Fossa
- Fat - a lot of it! - expansion
- Connective tissue
- Pudendal Canal
Pudendal Canal includes
- Pudendal nerve (S2-S4)
- Internal pudendal artery which branches into inferior rectal and external genitalia arteries
- Internal pudendal vein
