What are the 4 components with tetralogy of fallot?
right ventricular hypertrophy, pulmonic stenosis (causing right ventricular outflow obstruction), ventricular septal defect, overriding aorta
Is tetrology of fallot a cyanotic or acyanotic disease?
cyanotic
How old are these patients when they present with cyanosis from tet of fallot?
2-10 days
What will be heard on auscultation?
single second heart sound, systolic ejection murmur at left upper sternal border (maybe radiating to the back)
What are some other associations with tet of fallot?
cleft palate, hypospadias, skeletal malformations
Does oxygen improve this cyanosis?
no
What test can be done to check for tet of fallot?
Hyperoxia test–give 100% FiO2 for 10 minutes then get ABG…if paO2 is
What is immediate tx for tet of fallot?
give prostaglandin E1 to open the ductus
What can be done to treat tet of fallot until a full surgical repair can be done?
temporize with Blalock-Taussig shunt (BT shunt)
What is the most appropriate tx for a tet spell?
beta blocker
