Long- term mechanical ventilation (LTMV)
Long-term ventilator-assisted pts are defined by the American College of Chest Physicians (ACCP) as individuals require
MV for at least 6 hours for 21 days or more
Pts requiring LTMV can be divided into 2 groups:
- Those recovering from an acute illness and unable to maintain adequate ventilation for prolonged periods (COVID, PNA, heart attack)
- Those with chronic progressive cardiopulmonary disorders (ALS)
The overall goal of LTMV at home or alternative care sites is to improve the the pt’s quality of life by providing the following environmental attributes:
- Enhancing the individual’s living potential
- Improving physical and physiological levels of function
- Reduce morbidity
- Lessening hospitalizations
- Extending life
- Providing cost-effective care
What are the 3 sites for Vent dependent pts?
- Acute care
- Intermediate care
- Long-Term care
Acute Care Sites include
- ICU
- Specialized respiratory care units
- General Med- Surg care units
- Long-term acute care hospitals
ICUs provide
invasive monitoring, extensive care, and a higher ratio of practitioners to pts. (are more expensive)
Specialized respiratory care units have fewer resources and generally designed for
more stable VAIs.
Designated units may be called: pulmonary specialty wards and chronic assisted ventilator care units
These are typically located on general pt floors of the hospital. Nurses often require training to manage these VAIs
General Med-surg care units
These are designed to care for pts who require extensive monitoring and care such as daily Drs visits, continuous monitoring, IV therapy, wound car, and isolation
Long-term acute care hospital
Intermediate care sites include
- subacute care units
- long-term care hospitals
- rehabilitation hospitals
Long-term care hospitals are designed to provide care for
long-term invasive vent-dependent pts (i.e., length of stay usually exceeds 25 days)
Long-term care sites includ
- SNF
- congregate livening centers
- single-family homes
These sites do not have the resources to treat acutely ill pts
