Respiration problems RTN

BP higher/lower than normal
Difficult measuring BP
Cleanliness RTN
1-Equipment or furniture causes injury or isn’t working
2-Patient is storing unwrapped food

5-Bathroom area is not cleaned

7-Patient’s independence is obstructed bc of the room
Signs of complication of immobility RTN
1-Reddened skin usually at bony areas that does not return to normal color
2-urinate/bowel incontinence
Problems with ambulation (walking) RTN
1-Complaints of dizziness, shortness of breath, chest pain, palpitations, head pain
2-Changes in strength, ability, usual grip
3-Cooperative patient refuses to participate
Assisting with oral care RTN
- patient has dry red cracked bled lips
- cracked, loose, blackened teeth
- poorly fit, chipped dentures
- bad breath after oral care
- fruity smelling breath (sign of diabetes)
- red, white, swollen tongue
Assisting with perineal care RTN
- red, inflamed, rashes in perineal area
- unusual discharge
- bleeding from anus or vag
- abnormal color
Pressure ulcers RTN (4)
1-pale or reddened area with no normal color change within 5 min
2-previously reddened area is hot or painful
3-preciously reddened area now pale, white, or shiny
4-pressure ulcer change in size/depth
Hand care RTN (7)
1-pale/blue nailbeds 2-bruised nailbeds 3-unusual white/yellow nails 4-usual thick nails 5-broken nails or cut too short 6-torn/red/swollen cuticles 7-rashes
Foot care RTN (7)
1-pale or bruised nailbeds 2-yellow/white or thick or broken or cut too short nails 3-cracks or peels in the skin 4-ingrown toenails 5-unusual odors 6-itch or burn patient complaint 7-blister redness tender feet
