Communication
Exchange of information, the process of both giving and receiving
Communicating with someone who has speech difficulties
Ask yes/no questions if need basic info
Do any way to make them feel understood like using notepad or picture board
Answering call procedure
1-Good morning!
2-Identify self, “location, name, precession, speaking”
3-How may I help you?
4-If they need a person, ask to put them on hold, and do so
5-If they need an unavailable person then write down their message to the person, date, time, caller name, their number/email.
Observation
Something you notice about a patient
Objective
Information that can be obtained via measurements or visually such as temp or rash
Subjective
Information that cannot be objectively measure or assessed
Signs
Objective evidence of disease which are based on data obtained through measurements
Symptoms
Subjective evidence of disease and cannot be measured or observed by a viewer, a patient must tell you their pain
Reporting procedures
1-Write patient name and room#
2-Write down measurements such as vital signs if needed
3-Write everything in orderly matter
