- Expose the chest/torso

- Manual and visual sweep for penetrating chest wounds

- Treat all penetrating chest wounds on anterior of the patient

chest seal a Use gauze to clean around the wound b. Adhere during exhalation putting 1. Vented: put vent over wound 2. Unvented: ensure extends2” beyond wound on all sides c. Reassess intervention

- Log roll the patient and treat exit wounds

a. Use chest seals on the back the same way

- Monitor for s/s of tension pneumo

a. Severe/progressive respiratory distress
b. Severe/progressive tachypnea
c. Absent breath sounds on chest
d. Pulx ox <90%
e. Traumatic cardiac arrest w/o fatal wounds

A. Burp chest seal

i. Peel back seal,
ii. Wipe blood/clots
iii. Reapply on expiration

i. 2nd ICS mid clavicular line over the 3rd rib (OR) 5th ICS on anterior axillary line directly over 6th rib
ii. Choose needle 10-14ga 3¼ ”
iii. Insert over top of rib at 90 degree (to chest wall) and LEAVE NEEDLE in x 10 SEC
iv. Remove needle – leave catheter
v. Secure hub to chest
