• Premaxilla region most common 80%
• followed by mandibular premolar region
Aetiology?
• Genetic
• Local Factors - hyperactivity with the dental lamina
4 types of supernumarys?
- Mesiodens
- Tuberculate
- Supplemental
- Odontome
What can supernumeraries lead too?
• Displacement of erupted teeth
• Impaction
What are the clinical Associations?
• failure of eruption of other teeth
• Diastema - mesiodens
•Crowding, displaced teeth
• Root resorption
• prevention of tooth movement
• cystic change within supernumerary
What are the medical associations?
• CLP
•cleidocranial dysplasia
• gardeners syndrome (extra growth around the body)
Trt options for supernumeraries?
- Leave and montior to ensure no cystic change
- Extract and ortho trt
- Extract and expose permanent tooth if impacted
What is a conical supernumerary?
•Mesiodens - when in the midline
•75% most common
• peg shaped
•premaxilla
• complete root formation
What is a tuberculate supernumerary?
• Barrel/tube like
•12%
• premaxilla
• paired or multiple
• can stop upper incisors forming
• incomplete root formation
What is a supplemental supernumerary?
• looks like an extra tooth
•7%
• erupts last
• most common = maxilla lateral
• 2nd most common = mandible premolar region
What is a odontome supernumerary?
• mass of tooth tissue
• 6% rare
• 2 types-
1. Complex- irregular mass of tissue
2.compound- organised tooth structure
What is a supernumerary paramolar?
Supernumerary placed bucally or palatally
What is a supernumerary parapremolar?
A supernumerary which is buccally or palatally/lingually to the premolar
