Who owns the screening process and makes the suitability determination?
Unit COs of the transferring command
Military Treatment Facility (MTF) is responsible for what in the screening process?
- Performs medical/dental/educational screening
- Provides a suitability recommendation to transferring command
BUMED is responsible for what in the screening process?
- Prescribe procedures for medical, dental, and educational screening
- Monitor the execution of screening procedures
- Determine the cause and institute corrective action for screening deficiencies
Family Members require screening when:
- Orders authorize an accompanied tour overseas
- Member is serving an unaccompanied tour of 24 months or more
- Joining a service member already at an overseas/remote duty assignment
Suitability Screening Coordinator (SSC) Responsibilities:
- Designated by MTF commander
- Coordinates suitability screening process
- Performs paperwork review/preliminary screening
- Refers to MTF/DTF staff for screenings
- Forwards suitability inquiry to gaining MTF
- Provides recommendation to transferring command
- As required, coordinates to NAVPERSCOM, PSD, HRO, etc…
Suitability Screening Process:
- Service member receives orders (30-day clock begins)
- PSD schedules SM/FM screenings
- Transferring command conducts a “command review”
- SSC completes NAVMED 1300/2
- MTF/DTF conducts screening and completes NAVMED 1300/1 part I&II
- SSC forwards suitability inquiry to gaining MTF
- Gaining MTF assesses local resources and replies to screening MTF within 7 working days
- MTF CO/OIC provides recommendations to transferring command on NAVPERS 1300/16 part II
- Transferring command makes suitability determination on NAVPERS 1300/16 part I (30-day clock ends)
Required Information from service members:
- Copy of orders
- Demographic info for each FM
- Routine exams, screenings and immunizations
- Hearing and vision exams
- HIV, G6PD and sickle cell tests
- Blood type and DNA
- Copies of civilian medical, dental, or mental health records or summaries
- Current medical history (DD form 2807-1)

Current immunizations
Requirements specific for women:
Pelvic exams and Pap smear results, if over 21
Breast exam results:
Baseline mammogram at age 40 (35 with risk factors)
Screening mammogram every 1-2 years between 40-50.
Mammogram every year over age 50
Verbal inquiry for pregnancy screening
Requirements specific for infants and toddlers:
Copy of the current individualized Family Service Plan (IFSP)
If available, copies of developmental assessments or evaluations
Requirements specific for pre-school or school-age children
Copy of the current Individualized Education Program (IEP)
If available, copies of school assessments or evaluations
Copy of DD form 2792, Addendum B, completed by the school
Responsibilities of Suitability Screening Coordinator at the gaining MTF
Coordinates suitability inquiry with MTF/DTF HCP
Coordinates with HRO for civilians
Coordinates with local EDIS program and DoDDS for early intervention, special education or related services requirements
Forward reply by due dates
Submits screening deficiency reports and early return requests
Suitability screening cannot be completed when:
Assignment location is unknown
Information is incomplete
Final condition is not known
To identify and correct failures in the screening process, the gaining MTF files a report when:
Service/Family members arrive without medical, dental or educational screening
No coordination occurred with the gaining MTF although a special need was identified
A special need was not reported by the service member prior to the transfer
Screening records are incomplete, missing or do not contain required signatures
MTF responsibilities for suitability screening:
Performs the screening
provides recommendation to transferring command
BUMED’s role in suitability screening:
prescribe procedures
monitor screening
correct screening deficiencies
Family members require screening when:
- accompanied orders overseas
- unaccompanied orders <24 months
- joining SM already overseas
Suitability screening cannot be complete when:
- assignment location is unknown
- information is incomplete
- final condition is unknown
