Medical Recommendation for Flying or Special Operations Duty
What’s the purpose of DD Form 2992?
Notify CO or OIC that member is no longer medically recommended for flying duties
Who may issue an Aeromedical Grounding Notice?
All medical department personnel
Where is the Aeromedical Grounding Notice noted in the member’s health record?
-Special Duty Medical Abstract (NAVMED 6150/2)
Aeromedical Grounding Notices will be issued to the following:
- Personnel admitted to the Sick List
- Hospitalized
- Medical problem that could cause impairment during flight
- Immunizations, which require temporary grounding (Does NOT require issuance of an Aeromedical Grounding Notice)
When issuing “Down Chits”, who receives a copy?
- The original goes to the CO/OIC
- First copy goes to Ops Officer
- Second copy to Training Officer
- Additional copies for Service Member, Unit Safety or the NATOPs Officer
What’s the duration of a down chit?
Remains in effect until member has been examined by a flight surgeon and issued an Aeromedical Clearance Notice “Up-Chit”
Who has the authority to issue an “up-chit”?
-IDC
-PA
-MO
(if flight surgeon is not available; consultation must be obtained from a flight surgeon via email, verbal, or message prior to issuing clearance notice)
Who do you call if you issue a down chit?
If sickbay/BAS is detached from the unit, a courtesy phone call should be made to the CO, XO, and OPS advising them that a “down chit” has been issued for named individuals
All aviation personnel should be evaluated how often?

Who may issue aeromedical Clearance Notices?
- Flight Surgeons or Aviation Medical Examiners
- The authority may be given to an IDC, PA, or MO when in remote or isolated locations where a flight surgeon is not available.
- A consultation must be obtained from a flight surgeon, via e-mail, verbal communication, or message, prior to issuing the aeromedical Clearance Notice.

Aviation Mishap Investigation Procedures
- Search for survivors and treat all casualties first
- Collect medical records of crew and passengers
- Perform required PE and labs on all survivors
- Issue Grounding Notices
- Obtain statements
- Coordinate collection of survival gear and flight equipment
Coordinate with members of the aviation medical board in the collection of labeled sketches and photographs of crash site, to include:
Crash path Point of impact Wreckage Fatalities Photograph each survivor's injuries
Independent Duty Corpsman will gather all flight equipment and survival gear to include flight suits and/or uniforms.
- Photograph while intact and/or on person
- Label with name, SSN, rank, date of incident
- Identify all alterations you make, such as cutting off and moving
- Take Pictures of Flight Equipment and Survival Gear
- Pictures should be taken of the flight equipment, survival gear, flight suites and/or uniforms on the victims and survivors showing the condition of this equipment as worn is essential to the investigation; caution survivors not to remove equipment until you have taken pictures
- If the treatment of the patient requires that you cut off or remove the flight equipment and survival gear, document the alterations you’ve made

- Chronological Account of Activities of Previous 72 Hours (FORM SIR 3750/15)
- include food and beverages consumed

- obtain statements early
- be as clear and precise as possible to assist in the investigation

What are the physiological effects of OTCs on flight safety
- may require the need for medical assistance.
- symptoms of the effects may not be apparent until the individual is screened by qualified medical personnel with the necessary diagnostic equipment.
- Under no circumstances will an aircrew member be issued a clearance notice while on medication without concurrence from a flight surgeon!
What are the effects of Aspirin?
- Affects regulation of body temperature by acting on the hypothalamus
- Generates greater heat loss due to an increase in blood flow to the extremities and an increase in sweating
- Affects the acid-base balance of the body
- Causes a variation in the rate and depth of respirations
What are the effects of Nasal Decongestants?
- Insomnia, Fatigue, Headaches, Diarrhea
- Prolonged use increases blood pressure and pulse
- Dizziness, Confusion, Drowsiness, Amnesia, Possible depression
- Dry mouth, Euphoria, Decreased accommodation (inability to focus)
- Tachycardia, Diplopia (double-vision), Lassitude (weariness), Tremors
- Nausea/Vomiting, Decreased coordination, Gastrointestinal effects
What are the effects of cough drops?
- Depression of the CNS (Central Nervous System) with high doses
- Decreases reaction time
What are the effects of throat lozenges?
- Local allergic reactions are not uncommon
- Can result in decreased air
- Can result in rapid swelling of the oral pharynx and respiratory tract
What are the effects of Petroleum Jelly-Based Products?
- Becomes crusty when exposed to 100% O2
- Can cause discomfort with O2 masks during flight
