What artery is affected in classic stroke syndrome, where on it’s path (before or after internal capsule)?
The middle cerebral artery, before the internal capsule (the internal capsule is affected)
What are the symptoms of classic stroke syndrome?
Arm weakness
Facial paralysis
Speech is affected (due to facial paralysis (and hypoglossal paralysis)
Contralateral homonymous hemianopia
What tracts travel in the posterior limb of the internal capsule?
Corticospinal
Dorsal column and Spinothalamic
What tracts are in the genu (curve of internal capsule)?
Corticobulbar tract
What stroke syndromes have sparing? Where?
Partial anterior stroke syndrome (after internal capsule) (Lower limb sparing)
Anterior circulation syndrome (upper limb sparing)
What are the symptoms of middle cerebral artery stroke syndrome (after internal capsule) what structures of the brain are occluded to cause these symptoms?
Contralateral lower limb hemiparesis and sensory loss (PSC, PMC + supplementary motor cortex) With lower limb sparing.
Aphasia (receptive - wernicke’s, expressive - broca’s)
What areas of the brain are supplied by the posterior cerebral artery?
Optic radiations (meyers loop)
Hippocampus
Brainstem
In a non-brainstem posterior cerebral artery occlusion what symptoms may you have?
Homonymous hemianopia, with macular sparing
Some memory difficulty (if unilateral)
Anterograde amnesia (if bilateral)
What symptoms might you get in a posterior circulation syndrome affecting the brainstem? explain it?
Alternating hemiplegia - ipsilateral facial paralysis, contralateral long tract deficits
What might symptoms of a lacunar infarct be?
A specific tract deficit, (basal ganglia + internal capsule)
What arteries supply the internal capsule, what do they branch off?
Lenticulostriate arteries (lateral striate)
Branch off Middle cerebral artery
