Sports Medicine & MSK Disorders Flashcards
Splinting for tendonopathies for more than — weeks can cause significant loss of ROM
2
Ottawa knee rules
- > 55 yoa
- tenderness at fibular head
- isolated tenderness of patella
- inability to flex knee to 90 degrees
- inability to bear weight for 4 steps immediately after injury and in exam room
Ottawa ankle rules
- bone tenderness over posterior edge/tip of lateral malleolus
- bone tenderness at posterior edge/tip of the medial malleolus
- inability to bear weight immediately and in exam room
Ottawa Foot rules
- bone tenderness at the base of the 5th metatarsal
- bone tenderness at navicular
- inability to bear weight immediately and in exam room
Diagnosis and treatment of SLAP lesion
superior labrum from anterior to posterior tear. Common throwing injury, gives the sense of catching or popping of the shoulder with loss of force and pain. MRI to diagnose. First line therapy is insides and physical therapy. Surgical repair for severe symptoms that fail conservative management.
AC joint separation duration of feeling?
46 weeks for type one separation, about eight weeks for type two separation and up to 12 weeks with type three, may need surgery
Compressed and cubital tunnel syndrome?
Ulnar nerve
What nerve is compressed in carpal tunnel syndrome?
Median nerve
Treatment for mallet finger
Continuous extension splinting for at least six weeks followed by four weeks of nighttime splinting
- this is basically the opposite of jersey finger. If it anytime during the first six weeks finger is flexed this can cause a disruption in the healing so it’s important to keep the finger extended
Jersey finger treatment
Surgical repair
Diagnosis of scaphoid fracture’s
X-ray on presentation of snuffbox tenderness. About 10% of these will be negative initially so x-rays need to be repeated in 10 to 14 days to check for fracture. If tenderness persists however x-rays continue to be negative can get an MRI for further evaluation. Most of these will require surgical fixation
Side effects of chromium supplementation
Thought to increase lean muscle mass but can also cause liver and kidney dysfunction in addition to rhabdomyolysis. No evidence that there’s actually any benefit to the supplementing this
Lumbar Disc Herniation will start to improve in —- weeks and usually resolves by — weeks.
6 weeks & resolves by 12 weeks (usually)
4 risk factors for adhesive capsulitis
diabetes, parkinsons, cardiac disease & thyroid dysfunction
What the most sensitive test for carpal Tunnel syndrome?
Flicks Sign
Treatment of Plantar fasciitis
Rest, massage, stretching. Can use night split for 3 weeks(B) & OTC orthotics(B)
When would you refer a kid for developmental dysplasia of the hip?
if symptoms persist for 2 months
Club foot requires surgical correction at….
9-12m
MC bone tumor in kids
Osteosarcoma
MC BENIGN bone tumor in kids
nonossifying fibroma
Internal tibial torsion should resolve by age…
8
Femoral anteversion should resolve by age…
9-10
1 line and 2nd line for dx fracture? what is the 1st imaging is negative?
1st line xray. If neg but high suspicion repeat in 7-14 days. If still negative can get MRI w/o contrast
Treatment for clavicle fractures
figure 8 sling
Concern for saphoid fracture but negative xray..whats the next step?
repeat xray in 2 weeks - if displaced needs surgical evaluation
Treatment of jersey finger
surgical correction within 10-12 days
Treatment of boutonniere deformity
(aka jammed finger) splinting of PIP joint in extension for 6 weeks followed by night splinting for 4-6 weeks
Treatment of mallet finger
split DIP in full extension for 8 weeks
Most common nerve injured with distal radius fracture
Median N
Which two fractures require a CT to rule out fracture on?
Displace intra-articular distal radius fracture and tibial plateau fracture
For how long should a kid with mono avoid contact sports? When can he resume full activity?
3 weeks. 21 days after dx!
