Ceftriaxone/cefotaxime + Vancomycin + ampicillin
Periorbital/Preseptal cellulitis vs Orbital cellulitis
Periorbital/preseptal cellulitis - swelling of the eyelid, minimal pain, fever, NO visual defects, NO restricted eye movement
Orbital cellulitis - painful swelling of eyelid, conjunctivitis, opthalmoplegia, fever, visual defects, pain with eye movement.
CURB-65 scoring
Confusion, Urea >7 mmol/L, RR > 30, BP <90/60, age 65+
First line treatment for community acquired pneumonia in an adult who is otherwise healthy without comorbidities
Amoxicillin 1 g three times a day
Treatment of community choir pneumonia in an adult with comorbidities
Augmentin or a cephalosporin with a macrolide
* alternatively can do monotherapy with the respiratory FQ
Patients with what comorbidities need Pneumovax? What happens when they turn age 65?
Individuals with heart disease, lung disease, kidney disease, diabetes or smokers, liver disease need PPSV 23 now and then again when they turn 65
Patient is diagnosed with diabetes at age 62 and they receive their appropriate pneumonia vaccinations. What happens when they turn age 65?
Since they received PPSV 23 prior to age 65 their next dose would be given five years later. Do not give another PPSV 23 at age 65, wait until 67(5 years later)
