Proximal to the MTP joint of the big toe, inferior and proximal to the flare of the head of the Ist MT bone. (Roughly at the change in skin tone and texture.) The point lies in the nook underneath the Ist MT bone and above the muscle that spreads open the big toe (abductor hallucis), close up against the head of the bone. The depression is highlighted by rotating the big toe, or even more clearly, by the patient wiggling her own toe.

About I cun proximal to Sp 3, distal and inferior to the base of the Ist MT bone, between the bone and the muscle that spreads open the big toe (abductor hallucis). To be sure of finding the base of the Ist MT bone, locate the gap between the Ist MT bone and the medial cuneiform, and then slide your finger distally over the base, to the recess of the point. (As a rough guide slide your finger proximally from SP 3 along the medial edge of the Ist MT bone until you feel a notch in the bone, and then drop down into the seam between the bone and the muscle and feel for the point.)

With the person’s foot held at a right angle, draw a vertical and a horizontal line, in front of and below the ankle bone. The point is where the two lines cross. In order to needle this point keep your finger on the point and ask your patient to relax her foot.
