Somatisation and Medically Unexplained Symptoms
Persistent physical complaints but cannot find a problem with the body that could be the cause of this
Main concern- want to get a diagnosis to validate their experiences and find an appropriate treatment
Somatisation disorder
Recurrent presentation with medically unexplained symptoms
Affects multiple organ systems
Suggestible history
Non specific and atypical
Can be influential about your q’s so keep open q’s
Deny any emotional stress / are not registering the stress so it manifests itself physically
Personality disorder and substance abuse
Conversion disorder
Motor or sensory function Disturbance of normal function Not malingering- no secondary gain High rates of anxiety /depression Paralysis, non epileptic seizures (VBG lactate normal, no postdrome), loss of sensation, aphonia, amnesia, fugue states
Chronic Fatigue Syndrome
persistent or recurrent fatigue, ruled out other causes/conditions
Diagnosis of exclusion
Post exertion malaise
Factitious disorders
Munchausen’s syndrome
Intentionally fabricating clinical signs or symptoms
Exaggerate history
Can be comorbid with a personality disorder
Want to obtain medical attention and treatments
