Sketchy Path: Alcoholic and Non-alcoholic Fatty Liver Disease Flashcards
What enzyme turns alcohol to acetaldehyde?
Alcohol Dehydrogenase
What is reduced when alcohol converted to acetaldehyde?
NAD+ reduced to NADH
What happens in the liver when you drink to much alcohol?
All the NAD+ is reduced to NADH; No more NAD+ means you can not use it for fatty acid oxidation and fatty acids build up in the liver.
What happens to the acetaldehyde produced by alcohol dehydragenase?
Reduced to acetate (utilizing another NAD+)
Histology of fatty change to the liver looks like?
Hepatocytes fill with vesicles of fat
Microvesicular/Macrovesicular
What zone is first affected by fatty change?
Zone 3
centrilobular zone
What interleukin is responsible for calling in neutrophils?
IL-8
Which infiltrate is classic for alcoholic hepatitis and is usually absent in other forms?
neutrophils
What is a characteristic cell seen in alcoholic hepatitis?
Clumped intermediate filaments appear as pink densities and are term Mallory Bodies
Alcoholic hepatitis fibrosis starts where?
Centrivenular (Zone 3)
Which enzyme is greatest in alcoholic liver disease?
AST»_space;» ALT
What causes non-alcohol fatty liver disease?
Excessive visceral fat causes insulin insensitivity and altered fatty acid metabolism so the liver hold on to more fat
what is the major risk factor for disease progression in NAFLD?
Metabolic Syndrome
What do the liver enzymes look like in NAFLD?
ALT > AST
What is the triad seen in Reye’s Syndrome?
Encephalopathy
Microvesicular Fatty Changes in liver
Serum transaminase changes
