what do human factors encompass?
they encompass the understanding of the patterns and causes of error and systems failure, situational awareness, communication in teams and the limitations to human performance
why are human factors significant?
they are a major contributor to adverse events in heath care
what is the aim of clinical human factor groups?
to enhance clinical performance through an understanding of the effects of teamworks, workspace, culture, tasks and equipment and organisation on behavioural abilities and application of knowledge in clinical settings
how do clinical human factor groups work?
they work with clinical professionals and managers to make healthcare safer
what do individuals need to do for situational awareness?
they need to have their own situational awareness, awareness of the team, situational awareness within in a team to maintain the big picture, plan head and discuss eventualities and keep the team up to date with what is happening and how they will respond if the situation changes
what are the three conditions of situational awareness?
perception (what is going on around you), comprehension (understanding why), projection (what will happen next)
what are the characteristics situational awareness?
vigilance - preoccupation of failure
awareness - sensitivity to operation and shared goal and communication
engagement - commitment to resilience, climate of support and trust, motivated by ongoing development and empowering others
trust - deference to expertise and balance of skill mix
root cause - reluctance to simplify interpretation - is it relevant
what is the aim of situational awareness?
deal with conflict, review progress and reflect, encourage all team members to do best everyday
what do effective teams possess?
common purpose, measurable goal, effective leadership and conflict resolution, good communication, cohesion and mutual respect, situation monitoring, self monitoring and flexibility
what are the challenges in healthcare?
multiple handovers, environment that does not encourage speaking up, hierarchy, discouraging challenges and stress responses
what are safety briefings?
they are a handover ensuring all staff are aware off issues in order to protect staff and patients, they ensure smoother and safer operations and encourage all team members to speak up, it allows consideration of each case individually allowing for planning and problem resolution
what are the common human factors that increase risk?
workload, distraction, physical environmental and demand, device or product design, process design and teamwork
how can you improve situation awareness?
avoid reliance on memory, state the obvious, avoid ambiguous, make things visible, review and simplify process, standardise common processes and procedures, routinely use checklists, decrease reliance on vigilance and self awareness and well being
