The respiratory system contains which two kinds of membrane?
Mucous membrane
Serous membrane
What does the mucous membrane line?
The conducting portion of the respiratory tract. It bears mucus-secreting cells to varying degrees
What does the serous membrane (pleura) line?
The pleural sacs which envelope each lung
The serous membrane has what two layer?
Parietal serosa (outer wall) Visceral serosa (inner wall)
Why is the lung shiny?
It is covered in moist pleura
The two pleural layers have what in between them (the pleural cavity)?
Lubricating fluid
The pleural cavity may fill with what?
In each case state what this is called
Air- Pneumothorax
Blood- Haemothorax
Pus- Empyema
Watery transudate or exudate- Pleural Effusion
What is transudate?
Transudate is fluid pushed through the capillary due to high pressure within the capillary
What is exudate?
Exudate is fluid that leaks around the cells of the capillaries caused by inflammation
How can fluid be drained from the pleural cavity?
By inserting a wide-bore needle through an intercostal space (usually 7th posteriorly- below this there is danger of penetrating the diaphragm)
Done through ultrasound guidance
When a needle is inserted to drain the pleural cavity, how can you avoid the intercostal nerves and vessels that run along the inferior border of each rib, in the costal groove?
Insert the needle a fraction above the superior border of the lower rib
Cancer of the lung can cause what?
How?
Paralysis of one side of the diaphragm
By the tumours impinging on the left or right phrenic nerve
Do the lungs penetrate right down to the limits of the gutter margins?
No
What is a Pancoast tumour?
What can this tumour cause as a consequence of its growth?
A tumour at the apex of the lung
Wasting of the muscles of the lower arm as the tumour can impinge on the brachial plexus
What muscle wasting can be seen in the lower arm of some people with a Pancoast tumour?
The thenar eminence
Hypothenar eminence
Interosseus muscles in the palm
