What is the definition of sepsis and septic shock

A

Sepsis = systemic inflammatory response syndrome + infection

Septic chock = septic hypotension, doesn’t answer to adequate fluid

What is the mechanism of sepsis

A

First starts with activation of the immune system due to PAMP recognition, resulting in a cytokine storm and unspecific T cell activation. Foremost, resulting in a very high peak of TNFa, following MIF ( the main inducer of sepsis). Eventually causing vessels to leak so there is hypotension and hypoxia in tissues, causing multiorgan failure

What is the reason for invasive streptococcal infection

A

Invasive streptococcal infection is patients get toxic shock syndrome and can also get necrotic facitis

The reason for this is that streptococcal can produce super antigens activating 20% of all resting T cells
It also carry surface antigens that can be soluble and form complexes that cause sepsis
