Priority status epilepticus
First priority is stopping seizure
Start w fast acting ASD - benzodiazepine, propofol
Continue at constant rate infusion x 12-24 hr min
Continue long acting anti seizure med asap
Propofol
Use if 3 strike IV dose of benzodiazepine fails
- if seizure secondary to hepatic encephalopathy
- if seizure secondary to certain toxins (ivermectin)
Titrate to effect - 4-6mg/kg IV bolus
CRI - starting dose 0.1mg/kg/min
Intubation is likely
Ex of diazepam CRI
Seizures in genders
Common for females to have seizures when entering cycles of heat
Cluster seizures
More than 1 seizure in a 24 hour period
Concerning bc seizures cause seizures - need to get them under control
Drugs for cluster seizures
Intra nasal midazolam
- add ASD
- Keppra pulse tx
ASD for EI w cluster seizures
Benzodiazepines - Intra nasal midazolam & rectal diazepam terminate tansu epilepticus in 70% and 20% of cases - all dogs show sedation & ataxia
Add on ASD w cluster
If dog is on PB consider KBr
- most side effects
- likely most effective
- loading dose
Give t1/2 of KBr
If dog is on KBr consider PB
- similar side effects
- low starting dose 2.5mg/kg
Add on ASD Leve, zoni, gaba
Levetiracetam - common 2 or 3 drug
Zonisamide - common 2 or 3 drug
- chose over KBr due to side effects
- choose over levetiracetam to reverse leve for pulse therapy
