Scabies
Mites called Sarcoptes scabiei that burrow under the skin causing infection and intense itching
How long can it take for symptoms to appear after initial infestation
8 weeks
Presentation of scabies
Incredibly itchy small red spots,
Track marks
Between the finger webs
Management of scabies
Permethrin cream to patient and close contacts
Oral ivermectin - single dose for difficult to treat or crusted scabies
Wash bedding, curtains, towels etc.
Chlorphenamine at night - itching
How to diagnose scabies
Clinical
How to apply permethrin cream
Apply to whole body
Should be left on for 8 - 12 hours
Best to apply after shower
Should be repeated a week later as well
When does itching stop in scabies
Itching can continue for up to 4 weeks after successful treatment
Crusted Scabies
Serious infestation with scabies in patients that are immunocompromised
Presentation of crusted scabies
May not have itch - as no immune response
Scaly plaques often misdiagnosed as psoriasis
Management of crusted scabies
Oral ivermectin and isolation
