What are the clinical features of Acute sarcoidosis
A
- Sudden onset
- General: high fever, malaise, loss of appittie and wieght loss
- Pulmonary: chest pain, cough and Dyspnea
- Others: Artheritis, Anterior uvietis and Erythema nodosum
- Bilateral hilar lymphadenopathy

What are the features of chronic sarcoidosis
A
- Skin infiltrates
- Lung infiltrates
- Hypercalcaemia
- Peripheral lymphadenopathy

How do we diagnose sarcoidosis
A
- Chest X-ray - Hilar lymphadenopathy
- CT Scan - peripheral Nodular infiltrates
- Biopsy - gold standard
- PFT - Restrictive pattern
- Bloods - Raised IgG, Raised Calcium, raised inflammatory markers, Raised ACE

What is the treatment for acute sarcoidosis
A
- Usually none
- Give steroids if vital organ damage

What is the treatment for chronic sarcoidosis
A
- 1st line = oral steriods
- 2ns line = Immunosupressants - Azathryiopine, metothrexate. Anti TNF therapy
- Monitoring via PFT and CXR several years after due to relapse
- Lung transplant - last resort

What is the differential diagnosis of sarcoidosis
A
- TB
- Lymphoma
- Carcinoma
- Fungal infection
