A
Disease involving the terminal bronchi and alveoli
Impaired gas exchange, restrictive pattern

Aetiology
A
Disease of unknown cause
Less common in smokers
Common in afro-carribean’s
Multiple system involvement

Clinical Presentation
A
Acute - erythema nodosum, bilateral hilar lymphadenopathy, arthritis, uveitis, fever
Chronic - same as above + alveolitis (due to lung infiltrates), peripheral lymphadenopathy, hypercalcaemia

Investigations
A
CXR - diagnostic, look for bilateral hilar lymphadenopathy CT scan Biopsy PFT - restrictive pattern Blood tests - increased calcium

Pathology
A
Non caseating granulomas
type IV hypersensitivity

Management
A
Acute - self limiting, supportive treatment
Chronic - oral steroids, immunosuppression
