Impaired gas exchange, restrictive pattern

Aetiology
A
Most common
Common in smokers
Disease of unknown cause
Secondary causes - BREAST CA (bleomycin, radiation, EAA, asbestos etc)

Clinical presentation
A
Progressive breathlessness Dry cough Malaise Weight loss Symptoms gradually worsen

Clinical signs
A
Finger clubbing
Bilateral fine inspiratory crackles

Investigations
A
CXR - bilateral infiltrate
CT scan - key test. honey combing, fibrotic shadowing
Biopsy - not always needed

A
UIP (usual interstitial pneumonia)
Heterogeneous fibrosis in alveolar walls
Honey combing

Management
A
Oxygen
Lung transplant
Steroids and immunosuppressants don’t work
