When is hCG first detected?
-6-8 days after ovulation
When can the gestational sac be seen with a transvaginal ultrasound?
-when the hCG is 1500-2000
What does Biochemical pregnancy reger to?
-the presence of hCG 7010 days after ovulation but in whome menstruation occurs when expected
What does SAB even mean?
-spontaneous abortions
What is the most common cause of first trimester SAB?
- Chromosome abnormalities
- 45XO is most common
- trisomy is the most common class
What is the most common trisomy?
-trisomy 16
Threatened abortion
-vaginal bleeding and closed cervix
Inevitable abortion
- vaginal bleeding and the cervix is partially dilated
- loss is inevitable
incomplete abortion
- vaginal bleeding, dilated cervix
- passage of some but not all the products of conception
missed abortion
-the fetus is just sitting there in the uterus
What is the most common immunologic cause of recurrent (more than 3) abortions?
-antiphospholipid syndrome
Where is an ectopic pregnancy usually?
-fallopian tube
Who is at risk of ectopic preggo IF the pregnancy occurs?
-IUD users
Classic triad of ectopic pregnancy
- prior missed menses
- vaginal bleeding
- lower abdominal pain
What is the most common kind of ectopic pregnancy?
- possible ectopic preggo
- you just see a thickened endometrial stripe
- mild non specific symptoms
- the other types are probable and acutely ruptured
what will the hCG do with an ectopic preggo?
-double in 48 hours
What do we treat ectopic pregnancy with?
-MEthotrexate
When is the Rh thing a problem?
- in a pregnant lady who is Rh negative carrying an Rh positive fetus
- the antibodies from the mom cross placenta and get the child
- causes hemolytic anemia in the child
What do we give prophylactically to prevent maternal productoin of antibodies?
- RhoGAM
- it’s an Th immune globulin
- Anti D Ig: reduces availability of Rh antigens to mom
What can happen if we don’t take care of the Rh thing?
-fetal hydrops
