Appendix 3 of MEP gives guidance on consent and of the situations mentioned, only D would warrant breaking patient confidentiality. For D –MEP 3.2.2 states that children under the age of 13 are legally too young to consent to any sexual activity and instances should be treated seriously with a presumption that the case should be reported to social services unless there are exceptional circumstances backed by documented reasons for not sharing information.Schedule 1 CD –patient confidentiality should normally be maintained, and the police should be informed on the understanding that the source will not be identified (unless quantity larger than for personal use only where may be in public interest to identify source)

A 65 year old man has been admitted to hospital for investigations. he has type 2 diabetes. His blood glucose control has been poor and he has not been able o manage this with his medicine or lifestyle. He takes glicazide 160mg once daily and other medicines. The hypoglycaemic effect of antidiabetics is antagonised by thiazide diuretics, such as Bendroflumethiazide. The dose of the sulfonylurea (and in some cases any concomitant antidiabetic drug) may need to be reduced, to reduce the risk of hypoglycaemia.

If a 5 year old patient is taking
Phenoxymethylpenicillin 125mg/5 ml oral solution
Morphine sulphate 10mg/5ml oral soluition
Paracetamol 120mg/5ml suspension and hydroxycarbmamide oral solution. Which of the following conditions it the patient most likely to be experiencing
- Anaemia
- Folate deficiency
- osteoarthritis

None of the medications listed are indicated for anaemia or folate deficiency. Osteoarthritis is not seen in children.
The frequency of phenoxymethylpenicillin is for the prevention of pneumococcal infection in asplenia or in patients with sickle cell disease.
Hydroxycarbamide is indicated for sickle cell disease. The combination of hydroxycarbamide and phenoxymethylpenicillin indicate that the patient has sickle cell disease.
Morphine Sulphate and paracetamol are commonly prescribed analgesics in this patient group to help with sickle cell pain crisis
