Rickettsial Diseases Flashcards
name the relavent rickettsial pathogens and diseases
- rickettsia rickettsii - rocky mountain spotted fever
- rickettsia typhi - murine typhus
- erlichia chaffeensis - monocytic erlichiosis
- anaplasma phagocytophilum - granulocytic anaplasmosis
treatment for rickettsial diseases
DOXY BABY!
presentation of rocky mountain spotted fever
high fever, HA, maculopapular-petechial rash @ 3-5 days
normal WBC cnt w/left shift
thrombocytopenia
tick bite
pathology for rickettsia rickettsii
infection of endothelial cells
lymphohistiocytic vasculitis
blood vessel inflammation and damage resulting in loss of intravascular fluid
hypotension, end organ ischemic injury, cerebral and pulmonayr edema BAAAAAAAD
presentation and pathology of murine typhus
fever, HA, maculopapular rash on trunk
normal WBC w/left shift; thrombocytopenia
exposure to fleas instead of ticks; ww association w/rat infestation
presentation of monocytic erlichiosis and granulocytic anaplasmosis
fever, HA, myalgia, hepatitis, hypotension, end organ ischemic disease
low WBC count w/left shift; thrombocytopenia
AST and ALT released from liver into blood
tick bite transmission
pathology of monocytic erlichiosis and granulocytic anaplasmosis
infection of leukocytes; no vasculitis
geographic areas for:
erlichia chaffeensis
anaplasma phagocytophilum
erlicha chaffeensis - south central and southeastern US
anaplasma phagocytophilum - upper midwest and northeast US, northern california, europe
when to use rifampin for rickettsia?
children, pregnancy
complications of murine typhus
neuropsychiatric
CNS abnormalities - seizures, ataxia
renal insufficiency
jaundince
resp failure
differentiate progression of HME and HGA
HME can infect CNS, HGA cant
rash orrurs in HME but not usually HGA
