What is underlying pathology of Rocky Mountain Spotted fever and Murine typhus?
Infection of endothelial cells
Causes lymphohistiocytic vasculitis (blood vessel inflammation and damage) resulting in loss of intravascular fluid
What are complications of Rocky Mountain Spotted fever and Murine typhus?
Hypotension (Both)
End-organ ischemic injury (Both)
Cerebral edema (RM Spotted fever)
Pulmonary edema (RM Spotted fever)
What diagnostic techniques are used to make the diagnosis of Rocky Mountain Spotted fever and Murine typhus?
SEROLOGY And clinical (hx of exposure to tick or flea, fever, headache)
What organism causes human Monocytic ehrlichiosis (HME)?
Ehrlichia chaffeensis
What organism causes human granulocytic anaplasmosis (HGA)?
Anaplasm phagocytophilum
What is the clinical presentation of HME and HGA?
Fever, headache, myalgia (no rash)
In what locations is HME usually found vs. HGA?
HME - southeast and southcentral US
HGA - northeast and upper midwest US, Europe, East Asia
How can Rocky Mountain spotted fever and Murine typhus be differentiated from HME and HGA using complete blood count (CBC) results?
Rocky Mountain spotted fever and Murine typhus - NORMAL white blood cell count with left shift
HME and HGA - LOW white blood cell count with left shift
All exhibit thrombocytopenia
What is the underlying pathology of HME and HGA?
Infection of leukocytes; no vasculitis
