Rib attachments?
Attaches to superior costal facet of same number and inferior costal facet of above vertebra
What is costal tubercle?
Where rib articulates with the TP of the vertebra
What is costal angle significance>
Where you palpate rib posteriorly to look for SD
What ribs only articulate with their own vertebra, and not the one above?
Rib 1, 10, 11, 12
What are atypical ribs?
1, 2, 11, 12
What are true ribs?
1-7, they attach to the costal cartilage
What are false ribs?
8-10 they attach to costal cartilage of the rib above
What are floating ribs?
11 and 12 and they have no attachment
Where is sternal angle?
rib 2
Where does anterior and middle scalene insert?
Superior surface of 1st rib to elevate first rib
Where does posterior scalene insert?
2nd rib
Where does pec minor attach?
Anterior superior surface of ribs 3-5
Where does serratus anterior attach?
superior lateral surface of ribs 2-8
Where does latissimus dorsi attach?
Lower 4 ribs
Where does quadratus lumborum attach?
Inferior aspect of rib 12
Where does the neuro bundle run and what order?
VAN found between internal intercostal and innermost intercostal running along the costal groove on inferior side of rib
Bucket handle motion?
moves superiorly and laterally to increase transverse diameter of ribs 1-2, 8-10
What is pump handle motion?
Move superiorly and anteriorly to increase AP diameter, ribs 3-7
What is caliper or pincer motion?
Downward and posterior with inhalation Upward and superior with exhalation Ribs 11 and 12
What is torsional movement?
For example when T5 is rotated to left on T6
Left 6th rib:
- posterior aspect turns externally
- anterior extremity more flat with its inferior border sharp
Right 6th rib:
- posterior aspect of rib turns internally
- Anterior extremity having its superior margin excenuated
what causes torsional motion?
caused by articulation at inferior costal facet
Anteior subluxationn?
Both posterior and anterior parts of ribs are moved anterior
Posterior subluxation?
Anterior and posterior parts of rib move posteriorly
Examples of respiratory SD?
- Bucket handle ribs 1 & 2 & 8-10
- Pump handle 3-7
- Caliper 11-12
Structural SD examples?
- Torsional due to rotation of spine
- Non physiologic such as ant or post subluxation
What is inhalation dysfunciton?
- Rib being held in postion where inhalation is easy and restricted exhalation
What is exhalation dysfunction?
- rib being held in postion of exhalation so exhale is easy and inhalation is restricted
What is a key rib?
- First rib to be treated, it maintains a group of ribs in dysfxnl motion
- BITE
- Bottom rib is key with an inhalation dysfunction
- Top rib is key with exhalation
What does rib doesnt move in exhalation mean?
Inhalation dysfunction
What does symmetrical in inhalation indicate?
Inhalation dysfunction
What does it mean to say depressed rib?
Exhalation dysfunction
What does it mean to say inhalation restriction?
exhalation dysfunction
Causes of rib dysfunction?
- Thoracic scoliosis or kkyphosis
- Pectus excavatum/carinatum
- OA or osteoperosis
- Increased chest wall diameter
- abnormal tensions of shoulder mm
- Trauma to rib cage
- lifestyle
