What are polymyositis and dermatomyositis?
Autoimmune disorders causing muscle inflammation
Causing proximal muscle weakness
What is also present with dermatomyositis?
Skin changes
Gottron papules
Heliotrope rash
Periorbital oedema
Photosensitive erythematous rash on back, shoulders and neck
What can cause poly/dermatomyositis?
Cancer (makes them paraneoplastic syndromes)
Coxsackie virus
HIV
HLA genes
What test is used for myositis?
Creatine kinase
Inflammation of muscle cells releases CK
What can caused raised creatine kinase?
Myositis
Rhabdomyolysis
AKI
MI
Statins
Strenuous exercise
What is the most common antibody associated with polymyositis?
Anti-Jo-1 antibodies
How is myositis managed?
Assess for possible underlying cancer
Phyisotherapy
Occupational therapy
Corticosteroids first-line
Immunosuppressants, biologics second-line
