Define arthritis.
Chronic inflammatory disease characterised by potentially deforming polyarthritis and extra-articular manifestations.
What HLA groups are associated with RA?
HLA DR-1 and DR-4
How does RA typically present?
Symmetrical swollen, painful and stiff joints of the hands (MCP, PIP and wrist) and feet, as well as non-specific symptoms
What may be seen in late-stage RA?
Deforming arthropathy.
Rheumatoid nodules
Extra-articular manifestations
What is the best antibody to use in diagnosis of RA? How does this compare to rheumatoid factor?
Anti-CCP- more specific but similar sensitivity to rheumatoid factor
What radiological signs may be seen?
Erosions Periarticular swelling Loss of joint space Ostopenia Deformity
What functional rating scales can be used to monitor RA?
DAS28
How is the multidisciplinary team involved in the care of RA patients?
GP, physio, OT, nurse, specialist, podiatrist
What group of agents are first-line in RA treatment? What are the drugs of choice?
DMARDs. Sulfasalazine or MTX
What role do
a) analgesics
b) corticosteroids
have in RA management?
a) used as an adjunct- symptomatic relief
b) cover the lag phase of DMARDs, can help acute flares bt avoid long term use
