Rhabdomyolysis Flashcards
what is rhabdomyolysis
the end result of any diseas rpocess that causes muscle cell lysis, the muscle cell contents are release into the circulation and cause problems
which IC contents of muscle cells are potentially harmful
myoglobin, K, phosphate, urate, creatinine kinase
what can cause rhabdomyolysis
trauma, over exertional states, drugs
how do over exertional states cause rhabdomyolysis
direct muscle injury
which drugs can cause rhabdomyolysis
cocaine, meth, narcotics, diuretics
which cellula structure is disrupted in rhabdomyolysis
sarcolemma
how does rhabdomyolysis damage the kidneys
massive amounts of myohlobin are filtered through the glomeruli and precipitate obstructing the renal tubules
myoglobin is also nephrotoxic –> tubular cell necrosis
what colour does myoglobin turn urine
red bronw
does rhabdomyolysis present obviously?
it may eg after trauma, may also be insidious
which enzyme is raised in rhabdomyolysis
CK - levels correlate with severity
what is the main goal of treatment
prevent complications (mainly renal failure)
agressive rehydration!!!
why do we want to rehydrate patients
promote renal clearance of IC muscular toxins that have been released
desired urine output
maintain at 200-300 ml/hour
