The term “salt-sensitivity” refers to the increase in blood pressure that occurs when salt intake is increased.
In about 30-40% of hypertensive patients, there is a decreased slope of pressure-natriuresis indicating salt-sensitivity of blood pressure; during high sodium intake, larger increases in blood pressure are required to maintain sodium balance than during low sodium intake.
It is in these patients that even short-term reductions in salt intake over a period of a few weeks are most effective in reducing blood pressure.
However, in over 50% of hypertensive patients, there is a parallel shift of pressure-natriuresis, so that reducing salt intake has only a small effect on blood pressure, at least over a few days or weeks.
It should be emphasized, however, that salt-sensitivity is a continuous function that occurs to varying degrees in hypertensive as well as normotensive subjects. It is also important to note that in salt sensitive as well as salt insensitive hypertensive subjects there is a resetting or impaired renal-pressure natriuresis.
