Corneal ulcer- mod to severe

A

Zymaxid or Gatifloxacin 0.5% oph drop or moxifloxacin or ciprofloxacin q 2 h x 24 hr, then qid x 5 days
Prednisolone bid x 1week. Return 2 days if no improvement.

Also combine fluroquinilone + tobramycin or bacitracin or polymixin b

Can add cycloplegia to tx pain, prevent post synechia or if AC reaction present

Corneal abrasion + pain, w/o infiltrate

A

1 gtt 1% cyxlogel, 1gtt diclofenac in office, ciloxan tid x 5 days or

1% cyclogyl in office, poly trim qid, refresh pm qhs

Large central corneal abrasion from NON plant related trauma like fingernail

A

1 gtt atropine 5% given in office
Patch with polysporin ung overnight
Remove patch in AM at latest, and start tobramycin qid 3-5 days
Reaction 1 week or sooner if problem.

A

Maxitrol (neomycin polymixin b so4 and dexamethasone) oph solution qid x 1 week, bid x 1 week,
return 1week.
