What are the 4 types of records recorded at incidents?
Automated/system based records - IMS/BOSS
Statements - signed and witnessed (FIU/Police) usually after fatal fires.
Contemporaneous notes - To support writing statements within 24hrs, ie PRC’s/legal court.
Decision logs - hand written on CU’s CSS tool/Command support pumps.
When considering decision logging , a clear distinction should be made between?
Decisions to carry out an action that does NOT create additional risk to operational crews and which does NOT amend or change any current ops procedures, but has a significant impact on the public. (long term rail/road closures.
Decisions that lead to actions not covered by Ops procedures/policies requires additional safety measures to reduce risk.
Decisions logs should have the following what information?
Name of person making the decision. A decision number logged (maybe more than 1) Time of log entry made. Time action was, or is intended to be made/carried out. Location (sector 1/bridgehead etc) Description of action. Rationale underpinning the decision. Impact of carrying out the action. Risk assessment completed.
What form is a Key decision log made on?
Form 6182

Key decision log pad
Command unit decision log (gathering & thinking tool in CSS).
Key decision log on command unit (strategic development tool in CSS)
If a KDL has been made, what else MUST be completed?
Risk assessment & ranking matrix.
Post incident where do Key decisions get recorded?
IMS database.
Copies of KDL’s are held where?
One copy kept at station for 3 years then sent to Union St. All other copies sent in the first place to Union St.
