Cause of hypochromia
Cells have a larger central pallor due to lower Hb conc and a flatter cell
Interpreting red cell on blood count and film
- Is there anaemia
- Clues in blood count
- Clues in Hx
- Examine size, shape, colour
Iron deficiency anaemia
Checked Ferritin is reduced = low iron stores
Due to increased blood loss, insufficient intake, increased requirement
Clinical features: pallor, fatigue, breathlessness
Reduced Mch and Mchc
Iron deficiency
Reduced Mch normal Mchc
Thalassaemia trait
Hypochromia correlates to
Mchc
Thalassaemia trait blood count
Normal Hb
Low Mcv, mch
Mchc normal
RBC increased
HbA2 increased in beta thal trait
Ferritin normal in thalassaemia vs. Low in iron deficiency
Anaemia of chronic disease blood count
Hb low
Ferritin high
Low iron
High CRP
Iron deficiency transferrin
High
